Please complete the Chromebook loaner form below. This form must be completed for students to take home a device during the e-learning period. The device must be returned upon your child returning to school. Required fields marked with an asterisk *

Note: You do not need to pick-up a Chromebook if your student has access to a computer at home that can be used for school. Also, these devices require that you have Internet access. For more information on discounted Internet access for eligible students click on the link Comcast Internet Essentials, this is one of several options.

This form will be available until 6pm on Monday, March 30th, NO REQUEST will be accepted after this time. The cut-off is necessary to allow the staff time to have the devices ready for pick-up. All requests must be made using this online form.

Pick-Up Wednesday, April 1st from 1pm-6pm at Marya Yates Elementary School

There will be 3 doors used for pick-up:

Door 1 - Colin Powell Students (this is the main entrance of the school)

Door 2 - Sieden Prairie and Marya Yates Students

Door 7 - Neil Armstrong and Woodgate Students
