1. Print out the order form.

2. Choose a style from the price list.

3. Select the logo style on the shirt design form. Logo style #4 and #5 is available as a zip up hoodie and as a polo. Flannel pants will have the word EAGLES down the front leg.

4. Complete the order form with your item and logo numbers. Fill out the order form with your items and logo number.
5. Money must be turned in when order form is turned into Colin Powell.
